To obtain a passport you must appear in person and bring the following documents with you:
- Proof of Citizenship - A certified U.S. birth certificate, original naturalization certificate, or a previous passport. - Note: If your birth certificate says "Birth Registration", "Abstract", or "Hospital Certificate" it is not acceptable. You need to obtain a certified copy of your birth certificate from the State or County office of your place of birth. This will have a raised, embossed, or colored seal on it. A photocopy of the birth or naturalization certificate is not acceptable. You will receive your certified certificate back from the Department of State with the processing of your application.
- Photo Identification - Driver's license, State ID or military ID - Note: An interim driver's license is not acceptable.
- One (1) passport photo - 2" x 2" - Note: Photos can be obtained from several businesses including: Costco, FedEx, CVS, Target, Walgreens, WalMart
- Check or money order for $110.00 made payable to the Department of State - Note: The fee for a minor 15 years or under is $80.00. Cash or credit cards ARE NOT accepted for this fee.
- Separate payment by cash, check, credit card or money order for $25.00 (per application processed) made payable to the City of Roseville. (For expedited passports the additional $18.95 for postage raises this amount to 43.95).
- Minors - Special Conditions
- One Parent - If the application is for a minor aged 16 through 17, one (1) parent is required by law to appear in person with the applicant and countersign the application.
- Two Parents - If the application is for a minor under 16 years, both parents are required to accompany the child and countersign the application. - Note: If one parent is unable to accompany the child, a notarized signed affidavit and a photocopy of the driver's license by the absent parent must be provided. If a parent has sole custody, the original Court document showing this must be presented. If one parent is deceased, a certified copy of the death certificate must be provided.
- Birth Certificate - A certified birth certificate must be submitted for all minors under 16 even if they have a previous passport.
- You must have your (or your child's) social security number on the application. The IRS imposes a penalty if the social security number is not on the application.
- Standard Processing Time - It takes approximately six (6) weeks to receive your passport.
Note: If you are leaving in less than six (6) weeks, we recommend that you expedite your passport.
Expedite Processing Time - To ensure that you receive your passport in two (2) to three (3) weeks, an expedite fee of $60.00, plus a overnight postage fee of $12.72 must be included in your check to the Department of State, making it a total of $182.72 per expedited application for an adult 16 or over, and $152.72 for a minor under 16. In addition, a $18.30 overnight postage fee must be added to the City's processing fee making it a total of $43.30
Urgent Processing Time - For those with proof of a more urgent departure (itinerary or tickets purchased), an application should be submitted in person at the San Francisco Passport Agency located at 95 Hawthorne Street in San Francisco. Processing your request in San Francisco requires an appointment by calling (877) 487-2778. You must also present your itinerary or tickets for proof that you are leaving within two weeks.
